We recently received a W-9 that has an individual's name on the name line, an address and a SSN; the "Exempt Payee" box was marked. Should we take this as correct, or request a new one? Would this W-9 pass an IRS audit?

We spoke with IRS Information Reporting Specialist Mr. Siders (ID#1002856356) to confirm your assessment—an individual should not be checking an exempt box (and is this an older version W-9?). You should request a new W-9 from the individual, and refer them to the newest version W-9 form (December 2014) which asks for an exemption code and specifically states “codes apply only to certain entities, not individuals.”
