Per IRS Information Agent Ms. Crawford (#5506044) you may use a checkbox as authentication for signing Form W9. However, as part of the authentication, there must be a "penalties and perjury" statement as part of checking the box. The guidelines for this statement are outlined in the instructions. This ensures the company/person would be subject to fines and penalties if they provided you with false information. Access the W9 instructions and go to the section titled "Electronic Submission of Forms W-9."
